Password Scenarios:
1.If your name appears on the form as Mary May with an SSN of 711223456 then the password would be mary3456 (first 4 characters of your Full Name/Business Name Including spaces and special characters + last 4 digits of your SSN/ITIN number).
2. If your name appears on the form as AL D with ITIN as 517223355, then the password would be al d3355 (first 4 characters of your Full Name/Business Name Including spaces and special characters + last 4 digits of your SSN/ITIN number).
3. If your name appears on the form as AL & D with ITIN as 517223355, then the password would be al &3355 (first 4 characters of your Full Name/Business Name Including spaces and special characters + last 4 digits of your SSN/ITIN number).
4. If your business name appears on the form as A & D with ITIN as 012847934, then the password would be a & 7934 (first 4 characters of your Full Name/Business Name Including spaces and special characters + last 4 digits of your SSN/ITIN number).
